Our Newest Program: Care Advocate
Program
After a recent hospitalization of
her family member, Lucy realized
that even alert and oriented
hospitalized people need a care
advocate! The JCAHO (Joint
Commission for Accreditation of
Health Care Organizations)
recommends that all patients
entering a hospital have an advocate
with them to help ensure their
safety. We know hospitalized
patients, regardless of age, are
frightened about getting lost in
large health care systems, and feel
alone without an advocate. The
Patient Safety in American Hospitals
Study done between 2004-2006 found
in Medicare patients hospitalized
alone, there were almost 80,000
preventable deaths a year.
Our Care Advocate program, (a stand
alone or additional service
offering) provides trained advocates
to facilitate communication with
health care providers and family
ensuring the client wishes are
respected.
If a Care Advocate client is
hospitalized, their Care Advocate
works closely with the health care
team to ensure they are treated with
respect and the best care possible.
